PrEP (Pre-Exposure Prophylaxis)

 PrEP (Pre-Exposure Prophylaxis) refers to medications taken by individuals without HIV to significantly reduce their risk of acquiring the virus. When taken as prescribed, PrEP can lower the risk of HIV transmission from sex by up to 99%, and from injection drug use by at least 74%. 

Why It Matters

  1. High effectiveness: Daily oral PrEP regimens (like Truvada or Descovy) and long acting injectables drastically reduce HIV transmission rates  
  2. Empowerment & discretion: It gives people control over their sexual health—even when condom use is inconsistent or privacy concerns arise. 
  3. Essential public health tool: PrEP plays a pivotal role in national strategies to end the HIV epidemic  

Available PrEP Options

 🩺 Daily Oral Pills 

  • Truvada (emtricitabine/tenofovir disoproxil fumarate) – approved since 2012 for all individuals at risk  
  • Descovy (emtricitabine/tenofovir alafenamide) – approved in 2019 for people at risk via sex (but not for receptive vaginal exposure)  

💉 Injectable Options 

  • Apretude (cabotegravir) – FDA-approved in December 2021. An injection every 2 months instead of daily pills  
  • Yeztugo (lenacapavir) – the groundbreaking twiceyearly injectable approved June 18, 2025, with ~100% prevention efficacy in trials—a game-changer for adherence and convenience  

How PrEP Works & Who Should Consider It

 PrEP acts by interrupting the HIV lifecycle at exposure—preventing the virus from establishing itself in the body  

It's recommended for adults and adolescents who: 

  • Are HIV-negative and have had anal or vaginal sex in the past 6 months and: 
  • Have a partner with HIV (especially if their viral load is unknown or detectable), 
  • Don’t always use condoms, or 
  • Were recently diagnosed with another STI  
  • Inject drugs and: 
  • Share needles or 
  • Have an HIV-positive injecting partner  
  • Have used PEP multiple times or continue high-risk behavior  

Monthly HIV testing and regular check-ups are required to start and stay on PrEP safely.
